I don't have a good way to capture the QR (on android, the MyChart app doesn't allow screenshots and I didn't see a way to save/export). But I've re-generated the QR myself with our open source dev portal, starting with the shc:/... URI above:

This is a production SMART Health Card, which some folks had asked for examples of. It's my personal information that I'm releasing intentionally (also uploaded to https://github.com/jmandel/my-health-data/tree/master/my-data).

Note: I'm surprised to see "lotNumber":"UNKNOWN" for two reasons:

  • My paper card has lot numbers for each dose
  • If a lot number is unknown, our profiles leave this option so it should be omitted rather than populating with something other than a lot number (@Isaac Vetter I wonder if you can comment on this?)

Hey Josh - looking at your card, both vaccinations were received at Walgreens, and your SHC was issued by UnityPoint. Did you share your paper card with UnityPoint? If not, your data probably came from the Wisconsin immunization Registry, are you able to see if their system recorded a lot number?

I did not have a visit with UnityPoint or show them a paper card or request anything related to this. It just showed up in my portal (MyChart android app). I am not sure how to check with any of the underlying systems; I'm assuming that the lot# data got dropped at some point, and mostly I'm curious about how it got translated into the string "UNKNOWN" instead of being omitted.

But my main point is, I'm delighted to have access to my data in this format :-)

@Josh Mandel here is the link to the Wisconsin Immunization Registry. You just need to enter your sensitive identifiers and you will get a PDF of your immunization card -- https://www.dhfswir.org/PR/portalInfoManager.do

The form to access data worked (I mean, I'm not super thrilled that anyone who punches a SSN or insurance # into a state form can get all of a resident's history, but it worked) -- but there's no column for lot number in the results, so I can't tell whether one is present.

I can understand why lot number is important clinically... why is it important for a Vaccine Certificate? Seems use-case odd

It's an optional field. And if it's not known it should be omitted (but the behavior I'm seeing is: present with a value of "UNKNOWN").
