(2) shouldn't doseNumberPositiveInt and possibly seriesDosesPositiveInt be at least Must Support? Most of these vaccine are two doses and which dose you got seems a pretty important piece of information

On the other hand the VCI (US) profile _forbids_ dose numbers: http://build.fhir.org/ig/dvci/vaccine-credential-ig/branches/main/StructureDefinition-vaccination-credential-immunization.html#why-protocolapplied-is-not-allowed, so maybe not?

there are some complications on the dose number between the two use cases 'continuity of care' and 'proof of vaccination' in particular for travel. with RC2 we will have clarification on this.
