Relative refences mean that they are relative to the base of the resource created/updated. If a reference is to a resource on a different FHIR server, it must be a full URL.

Agree to use full URL. As we don't know exactly from which EHR/ordering supplier site when DME FHIR server receives PAO order request from DHR / ordering supplier.
