Hey Jens--maybe you've already gotten an answer for this in the past week, but I think the upshot is to use a PIVL_TS with @alignment="DW":

I'm not sure whether it's better to include the width or whether that would imply "all day every Wednesday" though.

When I asked the question I didn't realize that I was restricted to the use of IVL_TS (forgot to mention) at the time of posting

Since my use is in observation and not substanceAdministration, I can only use IVL_TS

Ah, gotcha. Yeah, I don't think there's any way to represent a periodic interval in an observation/effectiveTime.
