In the OrganizationAffiliation which out of the below data elements provided in the resource does the diagram organization points?

Since there are three Organization on the diagram but only 2 organization reference on the FHIR resource

Hi @Chetana Nayyar - Plan-Net Organization Affiliation references two organizations: participatingOrganization and organization. The PartOf is simply a reference from any organizaiton.

There is one challenge with this since OrganizationAffiliation as well as the PractitonerRole points to the same Organization block. This gives an impression that the organization to which a practitioner is associated even the participating organization is associated with the same Organization. Is that even a possible scenario ?

This drawing is showing relationships of PROFILES, not INSTANCES.
The number of times I repeated Organization is simply to make the relationships drawable.
It this more compact representation clouds that issue, I can go back to the previous form, and simple delete the edge that you noticed as a stray.
