Stream: Covid-19 Response
Topic: Status Updates
Abbie Watson (Jul 16 2020 at 19:27):
So, what's the status with SANER?
Do we know of any places that are rolling it out? Is Epic updating Bridges/Interconnect with software patches to roll out the MeasureReport APIs? Are Audatious Inquiry and other HIEs rolling anything out? NextGen or Microsoft? What's going on in everybody's respective worlds?
It's not much, but I'm mostly done putting together a proxy server for the City of Chicago and the State of Indiana, and have begun looking around for other state level APIs that we can consume and then proxy into MeasureReports.
Josh Mandel (Jul 16 2020 at 20:29):
I'm interested in this too, especially given the recent headline news on reporting. My hypothesis up to this week was... SANER would see uptake if CDC's NHSN would adopt it as a reporting mechanism. That no longer seems like a relevant assessment.
Keith Boone (Jul 17 2020 at 11:56):
The data being requested hasn't changed, and we are reaching out to HHS re: sending the data via States, but the players did change, so definitely a minor setback. I wouldn't count CDC as out of the picture over the long term, and they did come into the project in a big way before last week.
John Moehrke (Jul 17 2020 at 12:08):
note, it looks like IHE QRPH is going to be discussing their ADX profile use for COVID next week at the virtual face-to-face, tuesday morning 7am central time. Should be some SANER discussion.
Abbie Watson (Jul 18 2020 at 12:29):
So the SANER IG has already incorporated the data measures for Remdesivir and staffing needs?
Keith Boone (Jul 20 2020 at 19:07):
There is a staffing measure, there is not a supply measure for medications.
Abbie Watson (Jul 25 2020 at 01:40):
AMIA just released an open letter, and by their count there are 20 new data elements.
Carl Leitner (Aug 06 2020 at 16:20):
does anyone know if SANER will be using IHE's mADX profile of measure(report)?
Elliot Silver (Aug 06 2020 at 16:21):
@Keith Boone ?
John Moehrke (Aug 06 2020 at 16:37):
SANER could be seen as using mADX already. The mADX does not really add much constraints.
Carl Leitner (Aug 06 2020 at 16:50):
agreed mADX is pretty light, though does provide useful linkages for stratifiers/disaggregators as valuesets, particularly around group[].stratifier[].component[].code and relatedArtifact[].url
Abbie Watson (Aug 29 2020 at 14:55):
Okay.... has anyone sat down to parse the 20 new data elements from HHS into HL7 Measures yet? Should we be trying to accomplish that in the next two weeks prior to Connectathon 25? @Keith Boone ? @Josh Mandel?

As of last week, it may be back in play. I presented our work to a CDC subcontractor, and their comment was 'We were talking about the need for this [SANER] type of reporting 11 months ago; and here it is' and they're asking about feeding v2 interfaces from the LIS tracking network into SANER.
Keith Boone (Aug 31 2020 at 15:58):
Not yet, if you have a link to the new elements, I'd be interested in seeing what is there. I'm not likely to get anything done with it before connectathon at this date (I'm mostly OOO til Friday and then prepping for Connectathon).
Abbie Watson (Aug 31 2020 at 16:34):
I'll see what I can do to identify them and get a specific list. Also, it's great to see CDC NHSN participating in the connectathon. Great news!
Abbie Watson (Nov 21 2020 at 23:53):
Okay, I've been sitting in on HHS calls this week during the COVID-19 At-Anywhere Diagnostics Design-A-Thon, and found the specific lab reporting requirements from the CARES Act, and their HL v2 mappings.
CARES Act Section 18115
COVID-19 Pandemic Response, Laboratory Data Reporting
https://www.hhs.gov/sites/default/files/covid-19-laboratory-data-reporting-guidance.pdf
COVID-19 Lab Data Reporting Implementation Specifications
https://www.hhs.gov/sites/default/files/hhs-guidance-implementation.pdf
Going to use the CDCPatientImpactAndHospitalCapacity measure as a template, and create an HHSProtectCaresAct measure.
Terrie Reed (Nov 22 2020 at 03:01):
Note. The device identifier in the data elements should be the device identifier portion of the UDI linked to FDA’s GUDID. I’m happy to help with that part of the work

Ohai! We submitted the Symptomatic SANER sandbox servers as our Capstone Project, and invited everybody from HHS to participate in the next Connectathon! Please vote for our entry. Deadline is this Friday.

@Reece Adamson Reece - will the SANER Inferno server be available for Connectathon 26? I have folks from HHS and the WATERS repository asking about implementing FHIR infrastructure.
Reece Adamson (Jan 13 2021 at 20:03):
Hey @Abigail Watson the SANER IG has changed since the latest tests were developed and they don't necessarily reflect the updated API interactions for the IG (we currently have read and validate tests for the various profiles present at the time with a couple extra searches thrown in).
Our servers are pretty small so we weren't planning on having those tests actively hosted anywhere, but the code can be found here if anyone wants to run it locally: https://github.com/inferno-community/inferno-saner
https://inferno.healthit.gov/validator/ will be live and can be used for validating FHIR resources against the IPS profiles.
We won't have anyone dedicated to this specific track, but will have a few folks around to pop in and ask questions. Depending on how our other tracks go we'll try and drop in and run it live for a bit otherwise!
