Has anyone heard of work to capture and/or electronically exchange the content of the Person Under Investigation (PUI) form (https://www.cdc.gov/coronavirus/2019-ncov/downloads/pui-form.pdf)? I remember there was a question as to whether or not this should be built out as a Questionnaire resource, but I don't know if that went anywhere.

Most of the effort I've seen in the last week has been on standardizing sharing of metrics.

So... we actually started building a React App for this. We didn't get to encoding it in a Questionnaire or anything, but it's a pretty good start on the UI if anyone might find it helpful (and it's Apache 2.0 licensed). See: https://github.com/covid19-pui/covid-19-pui
It was a 1-week effort that is currently on hold. Documentation is minimal, but to run it, clone the repo, run yarn install
, and then run yarn start
. Full length screenshot below, but the screenshot obviously doesn't show any of the dynamic aspects of the form:

COVID-19-PUI-CRF.R4_v4.json See attached. I wouldn't put to much stock in my efforts though as it was the first questionnaire that I put together and could leave a lot to be desired

WRT to the PUI effort that @Chris Moesel mentioned. That could serve as the basis for a SMART app to collect the data, I dont think it would take to much effort to adapt it. It may also be helpful to encode the field names and values, currently they are simple string values

@Craig Newman @Chris Moesel @Rob Dingwell I was wondering if this "COVID-19 response" chat was tackling this PUI. Much more complicated data here! If there is support (meaning the effort will likely move along) I'd like to help out. In particular I'd hope this would be supported by CDC and ONC. Any chance of that?
