We have released a FHIR R4 IG with the questionnaire used in the open-source COVID-19 assessment web app CovApp. The questionnaire captures basic information like age and contact to known cases etc., as well as current symptom and basic risk factor information. The app was co-developed by the Charité University Hospital in Berlin and Data4Life and is in use in several countries, so I thought this might be interesting for the international community.
