BFH and Midata (@François von Kaenel ) are working on citizen-reported data for the open data community with an app for swiss-wide reporting of COVID-19 symptoms. The data will be available in FHIR format with SNOMED CT as semantics. An Implementation Guide is in development http://build.fhir.org/ig/hl7ch/covid-19-prom/branches/master/artifacts.html. Goal is to provide a first version at the hackatons coming up in switzerland. If you have any input, feedback do not hesitate to contact us.
