If you are following this track, we invite you to participate in the Care Coordination Track at the May Virtual Connectathon. You can sign up and view the schedule here: https://confluence.hl7.org/display/FHIR/2020-05+Care+Coordination+Track. We will be testing the development and exchange of #SDOH screening data, functional assessments, and person-centered plans. We are seeking implementers to generate clinical care plans using the FHIR CarePlan resources.
