On behalf of the Multiple Chronic Care (MCC) eCare Team, and Care Planning track leads @Dave Carlson and @Emma Jones, we want to invite care planning and clinical practice guidelines stakeholders to participate in the Care Planning Track which will evaluate and explore goal-oriented care planning. Once again reporting my Intro here. Dinesh Ganesan, Acarin (Digital Health Consulting Company). We provide a digital care management platform for care providers to create a personalized care-plan, engage with patients using a companion mobile app, and continuously monitor it using a real-time AI based monitor app. I would like to learn FHIR & care plan management as part of this connectathon.
