We're working with folks who are looking to adopt the patient summary requirements but aren't as familiar with reading the profiles in the snapshot / tree view.

I know most folks stay away from ERDs for the entirety of the FHIR schema - but has anyone had any luck in creating an ERD/ERD like diagram for any of the IPS profiles?

I presume Entity Relationship Diagram - sort of like a UML class diagram but simpler.

it's just a challenge because the diff view is.. misleading... and the snapshot is too detailed for a diagram

A "snapshot must support" view like the one in US Core might help.

Core includes a UML diagram for each resource. Of course they are unconstrained.

The IPS can have UML diagrams as part of the spec.
