Topic: FHIR Endpoint Directory
Richard Braman (FLY.HEALTH) (Apr 27 2021 at 14:28):
Many of you may be aware of the recent press release by CAQH https://www.managedhealthcareexecutive.com/view/caqh-launches-national-directory-of-fhir-endpoints-third-party-apps
FLY Health supports this idea. We have our own FHIR Endpoint directory at https://api.fhirfly.io/network/Endpoint . Right now this is the list of Epic FHIR servers (anyone who wants their organization added please contact me). Our question is who is the source of truth for this information? Who is the gatekeeper on which servers/apps are listed and which are not? For now we will keep building our own. But we call for a transparent no cost process to getting listed and some kind of system for replication of this data akin to DNS. Thoughts?

ONC and MITRE have built https://lantern.healthit.gov/?tab=about_tab which can take data from multiple sources including the official nppes provider directory system. These kinds of endpoint directory resources should be a public good
Ryan Howells (May 06 2021 at 16:36):
Thanks for your comments @Richard Braman (FLY.HEALTH) @Josh Mandel. At CARIN, we anticipate multiple private sector end point directories in the market. The ONC and MITRE end point directory provides an open framework for others to use. It's our understanding thought that the ONC/MITRE directory will not be proactively capturing payer end points. Unfortunately, the CAQH press release wasn't clear. It seemed to indicate that CARIN endorsed the CAQH end point directory as the only one payers should use. It should be noted that we did not approve the language in the press release before it was issued. It also should be noted that since we began, CARIN does not endorse any specific vendor's product or service offering. We are vendor, technology, and product agnostic. We always have been and always will be. Our hope is the industry can self-organize around a common approach for developing an end point directory the everyone can follow. We would also agree being listed in the end point directory and having access to that information should be a public good otherwise it creates perverse incentives in the market. @Richard Braman (FLY.HEALTH) we would welcome the opportunity to chat about the directory you have put together. Can you ping me offline so we can set-up a time to chat?
Richard Braman (FLY.HEALTH) (May 07 2021 at 23:28):
So I have some general questions I posted about using any collection of Endpoint references as a type of FHIR Directory. I think the idea is somewhat half baked. https://chat.fhir.org/#narrow/stream/179166-implementers/topic/Endpoint.20Resource
Ryan Harrison (May 18 2021 at 20:38):
I went ahead and created an HL7 wiki page to track the various endpoint point directories: https://confluence.hl7.org/display/FHIR/Endpoint+directory+implementations
Let me know if there is better place for this, or if there is an existing public listing I should add to.
Benjamin Langley (May 18 2021 at 20:43):
Is this now a directory for endpoint directories
Paul Church (May 18 2021 at 20:44):
I'll put that endpoint directory directory in my endpoint directory directory directory
Ryan Harrison (May 18 2021 at 20:52):
@Benjamin Langley and @Paul Church Yes, and believe it or not the meta humour was NOT intentional.
I've noticed a gap between the "common knowledge on FHIR chat" about the state of various implementations, and easy to read tables for i) new implementers and ii) stakeholders who don't care about the details and need a summary.
The directories of FHIR implementations and SMART on FHIR implementations are a bit out of date, but still great references. We're going to see an expansion of endpoint directory implementations over the next year, so why not start tracking.
Brendan Keeler (May 19 2021 at 13:02):
An endless series of tables pointing to other tables. Sounds like we need a referential database
Brendan Keeler (May 19 2021 at 13:02):
Jokes aside, love it
Ryan Howells (May 20 2021 at 19:33):
Great @Ryan Harrison! The CARIN application community put together a framework for what they would like to see in any endpoint directory. I posted it on your Confluence page. We would love the HL7 community's feedback either in this chat or directly to our team (@Mark Roberts @Ryan Howells) on what should be added or improved.
