What does this field mean? I take it to mean a reference to a dependent on the given coverage, but then why is it just a string instead of a reference to a Patient and why is the caridinality 0..1?

It is the identifier of the dependent, not the dependent itself - see http://hl7.org/fhir/coverage-definitions.html#Coverage.dependent
'For some coverages a single identifier is issued to the Subscriber and then a unique dependent number is issued to each beneficiary'
