How will this work with ballots? If the submitter's intent was to vote affirmative, and now it becomes a change request, will the submitter then have to withdraw a newly created negative?

No. The vote will carry and it'll still be an affirmative vote.

What you can't do is move a change request that has a negative vote to be a question, technical correction or comment - because none of those allow negative votes. (They also don't require votes to resolve them.)
