Operational definition for the session - data and metadata for who and when for the purposes for trust, traceability and identification.
- Information contained within a document may have its own "provenance" however, when this information is packaged into a document the document has a separate set of provenance metadata

Note: In the CDA context, as aligned with the standard - the author cascades for provenance, unless author is overridden in the body, or if a performer is identified (sorry for multiple attempts to add this note to the right stream)
