Header is one place...but as things got more complicated Structured Docs added a Care Team Section - http://www.hl7.org/ccdasearch/templates/2.16.840.1.113883.10.20.22.2.500.html

Oh the treacherous 'wild' -- the guidance came out in the last companion guide in support of USCDI v1 (2019). I suspect 'some' will support soon, since a few helped with design, and others will wait since not required by regulation. If/when USCDI v2 becomes a reality via SVAP (or a new reg), folks will have to shift to the Care Team section since the additional Care Team data elements required in USCDI v2 aren't supported in the CDA header
