Participant Check-list (Things to prepare before 1/9/2019):
1. A sample Encounter Summary Document (See slides for scenario)

a. Include a Section Time Range entry for each section
3. A C-CDA document with the following sections populated:
a. Results
b. Assessment Section, Plan Section, or Assessment and Plan
c. Medications
d. Procedures
e. Problems
4. A C-CDA document with clinical notes included
a. A separate Clinical Notes section in an Encounter Summary
b. A Note Activity entry in an existing section of an Encounter Summary
c. (optional, if you intend on supporting it) A Note Activity as an entryRelationship on an Entry in an existing section of an Encounter Summary
5. A sample Care Plan Document
a. Health Concerns with associated Goals
b. Health Concerns with associated Goals and associated interventions
c. Health Concerns with associated Goals and interventions, and with Health Status Evaluations and/or with Outcomes
d. With a Summary Section that shows the human readable view of the relates entries
6. A list of the CDA document entries your systems populates which you believe addresses the question 5W’s associated with Data Provenance
