Additionally, tests against my own server seem to fail randomly as well. Simply re-running the tests resolves the issue.

It looks like we have a race condition in the reference resolution tests. I've created a github issue to track this as we work on a fix: https://github.com/onc-healthit/inferno-program/issues/340
