@AbdulMalik Shakir For the jurisdiction valueset referenced in http://hl7.org/fhir/us/vrdr/STU1/StructureDefinition-Location-Jurisdiction-Id.html, namely https://phinvads.cdc.gov/vads/ViewCodeSystem.action?id=2.16.840.1.113883.6.92 is the appropriate codesystem to use in the CodeableConcept:

For the 56 jurisdictions (other than New York City): FIPS-5-2. 2.16.840.1.113883.6.92
For New York City: U.S. Board on Geographic Names (USGS - GNIS): 2.16.840.1.113883.6.245

Just confirming that there is no HL7 FHIR defining URL for either of these codesystems.
