In the Service Request, the encounter reference can remain as optional. This would account for a situation where a patient calls and mentions their child fell off a bike and has an oral trauma and needs a dental referral. In this case, there may not be an actual encounter visit.
However, there was agreement that at minimum the date/time the referral was made would need to be required. This may be captured in the Author.

In the Consult (Dental to Medical), should the "InFullfillmentOf" need to be required? Yes, for tracking purposes, need to tie this to when a referral was made (Team, please verify, let me know if still need to discuss this one).
