Hi all - here is a draft of the Alert for Admission and Discharge Resource Graph - comments appreciated. There two ways to link to a Condition in Encounter - Diagnoses and ReasonReference (reason for admission). I think either might be used - thoughts from ADT implementers/users? I'm also having trouble finding the content in the guide that documents what the proposed protocols are (description of use cases, roles of each actor, examples, etc). Can you share a more specific link?

This seems like the use case for FHIR Messaging. Have you looked at the MessageHeader resource?
