I am doing updates to FHIR core for DocumentReference, and find https://jira.hl7.org/browse/FHIR-26556
The resolution is by FHIR-I.
I am not exactly sure what the resolution effect would be.
One possibility is that this is only a change to Attachment datatype to add an extension that is a binary to indicate that the attachment is a thumbnail.
But I am not sure that is what is being indicated as the text seems to indicate the extension is of type Attachment.
