RE GF#22694 the resolution says nothing about n R4 standard extension but we surely could use one now. See this argonaut tracker. Can I apply this and if we created a standard extension for R4 when would that be usable?

I am usually doing this: https://www.hl7.org/fhir/versions.html#extensions, but then for a future version ;-)

so http://hl7.org/fhir/5.0/StructureDefinition/extension-CapablilityStatement.acceptLanguage assuming the R5 is correct and will work on the CI build...

Yes, that's what I would do. Though @Grahame Grieve I think has told me that we're bending the rules a bit here.
