Using FHIR#23738 for discussion on subscription changes. This comment lists the changes on the server as of right now (hoping to get more more pass in before Monday, but want to let people review prior to call).

I've gone through Jira to find issues relating to subscriptions, and I that's what I found. There are 3 issues are not in that list yet :

FHIR#25928 : adding Subscriptions to Exchanging Resources (in progress)
FHIR#22628 : websockets should support payload (have positive feedback from the model we tested, but haven't written it up)
FHIR#18993 : Subscription and Subscriber-side security (no discussions yet)
