The recommended search parameters do not allow "_include". This seems it would reduce the value of prefetched searches on MedicationOrder since at least one major EHR seems to use a medicationReference to refer to the med being ordered. Is there a way to handle this in prefetch, or does this use case always require the Hook to do follow-on requests?
