Should service designers aim to create globally unique ids (e.g., my-company-opioid-guidance vs. opioid-guidance)? Or is it enough that {baseUrl}/cds-services/{id} is globally unique?

No, only unique with a given base URL. We do not see a need to attempt to make service ids globally unique. Any attempt to do so would require coordination or UUIDs. The baseURL + service id is unique if a unique identifier is desired.
