Currently hook context documentation for order-review mixes FHIR resource types in a single json array / property.

Specifically, the order-review context states that a context.orders array is required and contains:

array DSTU2 - Array of MedicationOrder, DiagnosticOrder, DeviceUseRequest, ReferralRequest, ProcedureRequest, NutritionOrder, VisionPrescription
or
array STU3 - Array of MedicationRequest, ReferralRequest, ProcedureRequest, NutritionOrder, VisionPrescription

I think that each FHIR resource type belongs within its own array property.
