At 2:04:46 in the C-CDA IAT Day 1 video, someone (I guess @Jean Duteau), tells that FHIR mandates that a code system URN is always a URL except for the RxNorm. Where is that defined?

It wasn't me. John D'Amore and Rob McClure are speaking at that point and Rob says that in FHIR, you need to use the defining URL for that code system. Here are the relevant excerpts from the FHIR Specification:
http://hl7.org/fhir/terminologies.html: In FHIR, the Coding.system is always a URI - system: A URI that identifies the system
http://hl7.org/fhir/terminologies.html#system - gives the rules for determining the proper system

OK, so the standard talks about URI, not URL. I have not missed anything. Otherwise too many local systems are in jeopardy.
