The best discussion of composite measures is the STU4 release of the CQL-Based HQMF IG.

There is a draft of that content in the QM IG continuous build: http://build.fhir.org/ig/HL7/cqf-measures/composite-measures.html, which is a direct port of the content in the CQL-Based HQMF IG, but hasn't had a lot of cleanup applied yet. Composite measures are not yet fully supported within FHIR like they were within the HQMF/QDM-based measures.
