To supplement or optionally identify the location of a FHIRcast hub, a FHIR server MAY declare support for FHIRcast using the FHIRcast extension on its FHIR CapabilityStatement's rest element.

talked through with @bvdh during HL7 InM FHIRcast call, clarified that the intent is -- if the extension is used, then it's required and url is 1..1 to indicate it's a requirement for the fhir server and MS to indicate that clients must understand it.

The `fhir-resource` indicates the focus of the event; the `suffix` defines the type of event.

The `fhir-resource` indicates the context of the selection. The `context` element in a select event typically contains two fields. One with the name of the `fhir-resource` holding a the anchor resource and one or more named `select` indicating the resources that is selected.
