Thanks to Cindy Jiang for pointing out that there's some inconsistency in the specification around the open-patient-chart event.

The catalog and some examples document the event as open-patient-chart; yet, some descriptive text document patient-chart-open.

We should update the spec to consistently use a single string and review to ensure that we're consistent for all currently catalogued events.
