The description of author specifies the "he person or device that decided the described event should happen". we agreed to add device - but can the author ever be a care-team or organization, or is it constrained to something in a role?>

Organization is generally always substitutable. CareTeam seems less likely, but we don't require adherence to the 80% rule, so it's reasonable to include.
