I don't think it should be a 'wow'. It should be a normal, expected design consideration.

There are situations where it's legitimate for profiles to take it away and there are situations where it's not. Key thing is to try to make sure it doesn't happen unless truly appropriate.

and for those Resources that don't have an .author; the Patient can still author and this be indicated with a Provenance. Provenance can be used to indicate that the patient or RelatedPerson are author on any FHIR Resource.
