Twitter @GraceCordovano is a patient advocate in the traditional sense - she has many horror stories of data quality problems. We should tell people to anticipate this when they start to get their data .... meanwhile, this shows IMO why it's URGENT to get this data examined and fixed.

yes. I've heard of it happening here in Australia though it's probably not quite as frequent since there's no financial incentive
