A friend got vaccinated yesterday. The "receipt" they gave him listed the wrong arm. He asked them to fix it. Here's the bureaucratic burden they threw back in his face. ARG. Note that they said the info was "incomplete," not WRONG.
Thanks for your email. We sincerely apologize for the incomplete information on your vaccine receipt. In order for us to update your vaccination receipt information, can you please reply back with the following information (if known):
- Patient name
- Patient date of birth
- Patient email address
- Patient phone number
- Vaccine brand (Moderna or Pfizer)
- Administering Nurse
- Lot Number
- Vaccination Site (R/L arm)
- Dose Number (1 or 2)
- Expiration Date
- Vaccination Status (Received?)
- Adverse Reaction(s) (Y/N)
- Vaccination Location
- Vaccination Date
- Photo of the front and back of your Vaccination Card
Please refer to your dose one vaccine record card to help answer the above questions to the best of your ability. Once we have this information confirmed from you, we will reach out to the vaccination site directly and confirm any additional known details so that we can send you an updated vaccine receipt for your records.

On that note, thanks so much to all those contributing on the Patient Corrections stream in general. Even those of us not contributing much are learning a lot - this stuff ain't easy!
