I'm puzzled by the values in Observation Category.
Some codes are about the type of information : e.g. social-history and vital-signs, others are about the method of observation: e.g. imaging and laboratory.

1/ There is no category for calculated observations, such as Apgar score and BMI. ( BMI is also a vital sign.) Is calculation not an interesting feature of observation, so Apgar and BMI are both of category exam?

2/ The name 'survey' suggests it is primarily for info from things like questionnaires, such as those completed by a patient, e.g. last menstrual period, parity. Is it? If so the examples are wrong. If not, the code and display are misleading. Maybe call it something like 'formal-assessment'.

3/ I suggest that we add a category for obstetric history - observations about a patient's previous and current pregnancies, including the childbirth and postpartum periods.

4/ How would estimated gestational age be categorised. It can be calculated from LMP (maybe an exam?), derived from comparing ultrasound observations to a table of normal values (imaging?), assessed from an observation of the abdomen etc. (exam?), and others. What is the category when the value is produced by a clinician assessing and reconciling estimates made using different methods?

Although it is clear to me ( I wrote it :-)) instead of changing the name how about suggesting a clearer definition.

So the intended scope includes all of

  • assessment tools (e.g. Apgar Scores, Montreal Cognitive Assessment (MoCA),
  • questionnaires and surveys (e.g. patient experience survey),
  • forms (e.g. patient medical history on first visit to a clinical practice)
