I don't see a parameter to be able to say, "I''m searching in the context of a particular patient", where the patient's age, billing status, or institition may affect the type of tests that can be ordered. Has this kind of use case been considered?

It feels like there are two separate things being modeled there, one is the orderable itself, and the other is the protocol associated with ordering the item. Am I misinterpreting the intent?
