Observation.identifier says it is "A unique identifier assigned to this observation." Does that mean it is an identifier for the instance of the Observation in the patient's data (like a database record id), or an identifier for the type of observation (like a code)? If it is not like a code, is there any provision elsewhere in Observation or its extensions for storage of alternate or local codes (in addition to Observation.code)?
