Stream: Orders and Observation WG
Topic: Observation comments
Jose Costa Teixeira (Dec 22 2021 at 13:31):
Is there any guidance on adding to the Observation categorized notes such as
- [tech note]: this exam is not covered by accreditation
- [clinical note]: this exam may be affected by medications...
(sorry for poor examples - but I hope the idea is clear)
Jose Costa Teixeira (Jan 11 2022 at 16:21):
@Rob Hausam @Ralf Herzog
Jose Costa Teixeira (Jan 11 2022 at 16:21):
(not sure whom to ask)
Jose Costa Teixeira (Jan 11 2022 at 16:22):
I want to add an extension(?) that provides categorized notes - i.e. a note(text?) with a coded type
Jose Costa Teixeira (Jan 11 2022 at 16:22):
wondering if this has already been seen somewhere
Kevin Power (Jan 11 2022 at 16:31):
@Jose Costa Teixeira - #genomics is working on something just like this in our IG. I or @Patrick Werner can provide more details soon
Jose Costa Teixeira (Jan 11 2022 at 16:33):
thanks @Kevin Power
Jose Costa Teixeira (Jan 11 2022 at 16:34):
do you know where the genomics IG is?
Kevin Power (Jan 11 2022 at 16:34):
We have a branch with our proposal - http://build.fhir.org/ig/HL7/genomics-reporting/branches/pw-codedNotes/StructureDefinition-coded-annotation.html
Kevin Power (Jan 11 2022 at 16:35):
Hopefully you can start there. (Sorry but I am out and about and am only on my phone)
Patrick Werner (Jan 11 2022 at 16:38):
Hi @Jose Costa Teixeira yes i created an extension for this purpose (the one Kevin linked)
Jose Costa Teixeira (Jan 11 2022 at 16:38):
Nice, so you extend the data type
Patrick Werner (Jan 11 2022 at 16:38):
yes
Jose Costa Teixeira (Jan 11 2022 at 16:39):
i'm not sure if I should use it just because it's not yet official.. otherwise I make my own
Patrick Werner (Jan 11 2022 at 16:39):
we then just changed the target profile of DR.note
Patrick Werner (Jan 11 2022 at 16:40):
Jose Costa Teixeira said:

understandable. I wish this was part of the core spec (even as extension)
Patrick Werner (Jan 11 2022 at 16:40):
created a Jira Ticket about it in R3: https://jira.hl7.org/browse/FHIR-22102?jql=project%3D%22FHIR%22%20AND%20text~%22coded%20annotation%22
Patrick Werner (Jan 11 2022 at 16:41):
vote was on a later duplicate: https://jira.hl7.org/browse/FHIR-29705
(non persuasive)
Patrick Werner (Jan 11 2022 at 16:44):
@Grahame Grieve @Lloyd McKenzie
i can understand your points in the resolution. But now different people are starting to create similar Extensions to solve this problem.
Patrick Werner (Jan 11 2022 at 16:45):
Maybe at least a core extension would be an option to prevent wild extension growth.
Jose Costa Teixeira (Jan 11 2022 at 16:46):
actually I think my need is somewhere on Observation, but the requirement seems the same
Patrick Werner (Jan 11 2022 at 17:03):
you are right. We needed it on both.
Jose Costa Teixeira (Jan 12 2022 at 12:05):
so we're looking at a "TypedAnnotation" data type, that contains that extension?
Kevin Power (Jan 12 2022 at 13:46):
We called it "CodedAnnotation", but yes.
Rob Hausam (Jan 27 2022 at 18:34):
@Jose Costa Teixeira I'm coming to this late. Has this answered your original question?
Jose Costa Teixeira (Jan 27 2022 at 18:51):
Yes
Jose Costa Teixeira (Jan 27 2022 at 18:54):
I do think it should be a standard extension
John Moehrke (Jan 27 2022 at 18:56):
I would prefer they be a Resource that points at the Observation it is commenting on.
Jose Costa Teixeira (Jan 27 2022 at 19:32):
@John Moehrke wrong thread? This is about mapping NTE-4 by adding an annotation type to Annotation
