Topic: Extension for risk on a thing

view this post on Zulip Eric Haas (Mar 31 2020 at 00:27):

We need a generic risk extension for Specimen (and possibly for Biologic Product and Procedure) that can handle various labels, levels (infectious risk, aerosol-generating procedure, radioactive, etc.)

would this be a reference to Flag?

view this post on Zulip Lloyd McKenzie (Mar 31 2020 at 00:30):

Typically Flag points to the thing flagged rather than the reverse. Right now, Specimen hasn't been identified as a subject type, but it certainly could be. Does it make sense for the risk flag to be maintained independently or should it be a property of the extension directly?

@Riki Merrick are there handling and risk elements in the Specimen DAM that are planned for addition?

view this post on Zulip Eric Haas (Mar 31 2020 at 00:32):

These sound like generic labels like a caution sticker on Mrs Smith's cat Mittens chart. so flag is probably overkill.

view this post on Zulip Eric Haas (Mar 31 2020 at 00:36):

So a caution extension or a handling extension with valueCodeable to describe the peril is the simplest solution

http://hl7.org/fhir/StructureDefinition/specimen-specialHandling already is there for Specimen will see if:

  • that covers it or needs something more - FHIR on OO Call
  • whether the context is too narrow and should be more generic for any "thing". - discuss here

view this post on Zulip Eric Haas (Mar 31 2020 at 00:40):

I suspect handling is too narrow for generic risks and need something similar for that but they are tied together.

view this post on Zulip Riki Merrick (Mar 31 2020 at 02:56):

Funny you should say that - we were just talking about a generic risk that
might be needed for specimen - it is in the DAM; biologicallyDerivedProduct
and procedures - see minutes from today:
https://confluence.hl7.org/display/OO/Specimen+Call+Minutes+March+23%2C+2020

the v2 vocab is focused on specimen, but for procedure we would need
something to include aerosol creating at the moment.

I thought Eric's comments were probably in response to our discussion today - even better that they were independent but in concert with it.
