On the build site, the observationLinkPeriod page says the uri is http://hl7.org/fhir/uv/sdc/StructureDefinition/sdc-questionnaire-observationLinkPeriod. However, LHC-Forms has for some time been using http://hl7.org/fhir/StructureDefinition/questionnaire-observationLinkPeriod. I recognize that the build site content is sometimes superseded by jira issues, so I am not sure what the really current thinking is about what the uri should be.

I think the one you're using is from Grahame's blog proposal. The sdc one is the place it's officially defined. Given the tight coupling between the extension and the documentation of its use in SDC, I don't see that changing any time soon (or ever).

Nope. That one was http://hl7.org/fhir/StructureDefinition/questionnaire-observation-link, which was changed in J#17665 to be "questionnaire-observationLinkPeriod". In September of 2018 we had the URI I am using now. (See http://hl7.org/fhir/2018Sep/extension-questionnaire-observationlinkperiod.html.) I will update to the new one, and I guess I will need to support both the Sep. 2018 one and the current one to be kind to users.
