If a Questionnaire has an item of type quantity but provides neither questionnaire-unit nor questionnaire-unitOption/ValueSet, should a renderer provide a free text field for the unit?

It could, and/or a select box that leverages a value set of its own choosing - most preferably UCUM

UCUM is compositional. I am not sure the typical user would know the multiplication operator.... But I suppose something could be done along those lines.

Yeah. I mean, for you, if there was a LOINC code you could infer. Or just a default list of most common ones + text

That's what I would do-free text unit (but don't handle this case yet)
