The new NLM form builder (https://lhcformbuilder.nlm.nih.gov/app/beta/, still in beta with some issues we're fixing) is Angular-based. It is open source, at https://github.com/lhncbc/formbuilder-lhcforms. But, I am not sure if you are just looking for re-usable components to use in your own form builder. I suspect you won't find something like that in our repository; it is not something we planned for.

You may want to take a look at Questionnaire to Survey which leverages SurveyJS and should work with angular.
