I’m working on allowing Questionnaires to be retrieved through our server, and I’m needing to expose information for the maximum decimal places allowed for an item.item. I found this extension http://hl7.org/fhir/r4/extension-maxdecimalplaces.html which is what I need, but it says it can be used for Questionnaire.item and does not mention Questionnaire.item.item (other extensions have called out Questionnaire.item.item specifically) Can this extension only be used on a top level item?

It should be useable at any layer - that's certainly the intention. If you run into validation issues, let us know.
