I think that is outside the questionnaire (or at most an extension) and should be a part of the host application that renders the form.

The simplest is to have a question at the beginning that asks the user to self-identify their category, then have enableWhens on subsequent questions that are driven by the answer to that question.

My question is more about access to the Questionnaire on a FHIR Server. I think is more of a scopes question.
