Is there any step by step guide for SDC implementation available somewhere ? I would also love to hear from those of you who implemented SDC before and willing to share your experience, or a high level summary of the required pieces.
I am currently authoring the questionnaires using Grahame Grieve and NLM tools.

I assume you've seen http://hl7.org/fhir/uv/sdc/2019May/ ? I think it depends on what you are trying to accomplish.
