If there is an item that is a question that repeats, my understanding is that for the Questionnaire response, there should be just one item that has an "answer" entry for each answer.
However, I don't see anything on the QuestionnaireResponse page that says the data must be represented that way. It looks like one could also create a QuestionnaireResponse that had one item for each answer (where each answer had one "answer" entry). Should a constraint be added, or is that also an acceptable format?

I don't think we can enforce it as a constraint, but we should absolutely make clear that's the rule
