Is anyone building questionnaire authoring tools? Either open source or commercial efforts in this area?

@Grahame Grieve has one that's open source and @Brian Postlethwaite has a commercial one. I believe the NLM is producing an open source one as well.

NLM's form (questionnaire) builder is at https://lhcformbuilder.nlm.nih.gov/. It has a tab that lets you see the questionnaire definition for the form you are building with the GUI, and also has an option (under Export) to send the questionnaire to a FHIR server.
