It seems in the questionnaire I use a code to map the answer to a field. Where is a list of the FHIR specific codes? for example. Patient.name?

They aren't codes, but rather paths. The current documentation on the various mechanisms to do this (work in progress) is here: http://hl7.org/fhir/us/sdc/STU2/
